Position Purpose/Summary

The Community Support Specialist delivers direct care support, assistance and training to people with developmental disabilities, and/or TBI/Dual Diagnosis to consumers who live in various community settings. He/She encourages consumer to be involved in their community. ACTT Nurse

Position Purpose/Summary

The Nurse will function as a clinical member of the ACT Team, providing needed nursing services, psychotherapy, and support services under the supervision of the team leader and/or the team psychiatrist. The ACTT Nurse is responsible for conducting psychiatric assessments; assessing physical health needs; making appropriate referrals to community physicians; and managing and administering medications in conjunction with the psychiatrist. The Nurse (along with the entire team) is expected to help consumers avoid re-hospitalization and episodes of crisis care; achieve recovery from mental illness/substance abuse issues; meet basic sustenance needs; and improve their overall quality of life, per consumers’ report.  Although all services provided are included in the person-centered-plan and performed under the supervision of the ACTT program manager and psychiatrist, such supervision does not involve immediate oversight of all job functions.  Therefore, this employee conducts most of his/her work independently with a high degree of freedom to act based on clinical judgement.

Position Purpose and Summary

The Lead Clinical Specialist (LCS) provides clinical guidance, behavioral intervention and supervision to individuals and families in population served. The LCS has primary responsibility for behavioral intervention management and coordination of services for the individual. The LCS works intensely with the care provider by providing ongoing support that serves to stabilize the placement and facilitate behavioral change necessary for successful placement. This includes facilitating the setting of placement goals and desired outcomes established by the placement team. The LCS is responsible for writing and monitoring behavioral intervention treatment plans, monitoring progress according to the level of care, and documentation and planning transitions for discharges.

The Disaster Case Manager will provide long term disaster case management services to program eligible survivors of natural disasters. Director of Programs- Refugee and Immigrant Services

Position Purpose/Summary

The Director of Programs will manage and oversee the operations of Refugee Resettlement in Raleigh, NC in addition to assigned Immigrant Services programs in NC/SC. Post Release and Home Study Case Manager- Immigrant Services

Position Purpose/Summary

Lutheran Services Carolinas’ Refugee and Immigrant Services Department provides culturally appropriate, trauma-informed, comprehensive services to refugees and immigrants across North and South Carolina.The Post Release and Home Studies Program Case Manager works with Unaccompanied Children (UCs) who are fleeing their country of origin and arrive in the U.S. without a parent or legal guardian. UCs are placed into the custody of the Office of Refugee Resettlement and often released to caregivers in local communities across the country. Lutheran Services Carolinas works in collaboration with the Office of Refugee Resettlement and Lutheran Immigrant and Refugee Service to provide case management to UCs who are reunified with caregivers in North and South Carolina.

Foster Care Case Manager

Position Purpose/Summary
The Case Manager (Therapeutic Foster Care or Unaccompanied Minor program) has primary responsibility for behavioral intervention management and coordination of services for the youth.  The Foster Care Case Manager works intensely with the client and foster parent/care giver by providing ongoing support.  If applicable within program, also serves as a supervisor and consultant to licensed therapeutic foster parents (contractors) who carry out the therapeutic interventions for the youth in care.
