LSC follows a person-centered model of healthcare called New Pathways. New Pathways encourages the development of resident-centered practices and a culture that emphasizes an individual’s independence and overall quality of life. In essence, daily decisions, both large and small, are left up to the residents of our communities as much as possible.

The intensive New Pathways training program creates more capable LSC staff members, giving them the competencies and compassion to make the best decisions for the residents in their charge.

Our Service Areas
Adult Day Services
Adult day service programs, including LSC’s Trinity Living Center in Salisbury, help end the social isolation that adults can experience due to changes in health, resources, or mobility. Participants enjoy activities and establish friendships while receiving medical care, nutritious meals, and assistance with personal care if and when it’s needed.
Caregiver support and respite are another fundamental part of adult day services. It’s not easy taking care of a family member in need. Adult day services provide an alternative to assisted living or a live-in senior community. Families know their loved ones are safe and secure while they’re able to work, rest, or run errands.

Trinity at Home is a program that offers home care in Rowan County. Based on the Trinity Oaks campus, Trinity at Home provides a person-centered approach that gives seniors the freedom to remain comfortably in their own homes with the care and support they need.

(Program of All-Inclusive Care For The Elderly)
PACE is an innovative Medicare program and Medicaid option for adults 55 and over who need skilled nursing care but wish to remain at home. The PACE focus is on prevention, early intervention, and wellness, resulting in overall health improvements and cost savings.

Affordable Housing
Lutheran Services Carolinas assists seniors in securing safe and affordable housing. Our organization operates Dallas High School Apartments in Dallas and Lutherhaus in Hickory, adjacent to Lenoir Rhyne University. Both facilities offer subsidized housing options tailored for older adults and individuals with disabilities.
